or request a transfer.

 

Attention: New prescriptions cannot be ordered using the form. Prescription requires medical contact. For certain transfers, contact with a doctor is also necessary beforehand.

In this case, please make an appointment first.

The insurance card must have already been scanned in the current quarter or must be presented when picking up.

The ordered prescriptions/transfers can be picked up at the health center on the next working day.
